The Span Center helps older adults and their families create personalized care plans to accommodate individual needs and preferences. Our care coordinators assess each situation, connect clients to necessary resources and collaborate with community organizations and service professionals to ensure individuals receive the care they need. We also offer guidance for caregivers and follow up to ensure services are meeting expectations.
As part of our Care Coordination service, we also assist Medicare beneficiaries in accessing eligible programs to maximize savings and improve quality of life. Our trained staff helps clients determine eligibility and apply for benefits at no cost. Programs include:
- Extra Help with Medicare Prescription Drug Costs
- Medicare Savings Programs (MSPs)
- Medicaid
- Supplemental Nutrition Assistance Program (SNAP)
- Energy Assistance (Fuel, Crisis, Cooling)
- Weatherization
- Additional Benefits (e.g., eyeglasses, eye exams)
Care Coordination is available to individuals 60+ who reside in the City of Richmond or the counties of Charles City, Chesterfield, Goochland, Hanover, Henrico, New Kent, and Powhatan. There are no income requirements, but income may be assessed for service eligibility.
